Medical Approval Statement

We (I) authorize the camp director or his designee to secure medical or surgical treatment as recommended by a physician for the applicant’s wellbeing. The camp health director or nurse may administer any prescribed medications and treat any emergency that may arise while the applicant is at Rockford Christian Camp.

I hereby grant to Rockford Christian Camp, Inc. all rights, title, and interest in and to any and all photographic images and video or audio recordings made during my volunteer activities with the organization, including but not limited to any royalties, proceeds or other benefits from such photographs or recordings.
